Multi-visceral Abdominal Trauma
Multi-visceral abdominal trauma refers to a serious abdominal injury involving two or more internal organs. It commonly occurs following high-energy blunt or penetrating trauma and may involve the liver, spleen, pancreas, bowel, stomach, kidneys, blood vessels or other abdominal structures.
Because multiple organs may be injured simultaneously, patients can develop internal bleeding, contamination of the abdominal cavity, organ dysfunction and shock. Symptoms
Symptoms depend on the organs injured and the severity of the trauma. Common signs and symptoms may include:
- Severe abdominal pain
- Abdominal tenderness
- Abdominal swelling or distension
- Nausea and vomiting
- Weakness, dizziness or fainting
- Rapid heartbeat
- Low blood pressure due to internal bleeding

- Rapid stabilization and resuscitation
- Intravenous fluids and blood transfusion when required
- Close monitoring in an intensive care setting for severe injuries
- Non-operative management for selected stable solid-organ injuries
- Angiographic embolization for selected ongoing arterial bleeding
- Emergency exploratory laparotomy for uncontrolled bleeding or significant abdominal contamination
- Repair or resection of injured bowel and other hollow organs
- Liver, pancreatic or splenic procedures according to the specific injury
- Management of associated vascular, urinary or other organ injuries
Patients with hemodynamic instability, uncontrolled bleeding or major hollow-viscus injury may require emergency surgery. Stable patients with selected injuries may be managed with close observation and minimally invasive or interventional procedures.
The treatment strategy is individualized according to the overall injury pattern and requires coordinated care between trauma surgeons, surgical gastroenterologists, anesthesiologists, radiologists and critical care specialists.
Prevention
Multi-visceral abdominal trauma cannot always be prevented, particularly in major accidents. However, appropriate safety measures can reduce the risk and severity of traumatic injuries.

- Use appropriate protective equipment during sports and high-risk activities.
